Community Choice Michigan is a nonprofit Medicaid & Medicare managed health care plan that has served consumers in Michigan for 11 years. Currently, through our state contract, we serve more than 49,000 Medicaid members in Michigan
The plan is sponsored by a network of 16 Federally Qualified Health Centers, or nonprofit community clinics that provide health care to underserved people.
Community Choice Advantage is a special needs health care plan committed to helping you get the care you need. Community Choice Advantage is a plan for those who are eligible for Medicare Part A and Part B, as well as Medicaid.
Community Choice Advantage is a Health plan with a Medicare contract that is available in 6 Michigan Counties:
Alpena, Genesee, Kalamazoo, Saginaw, Van Buren, St. Joseph
